How to Apply

Dear Parent/Guardian:

     In order to start your child’s enrollment, the necessary documents are needed to be on file. Please bring in all original documents. We will make copies at our office for the applicant's file.

1. Birth Certificate (from Dept. of Vital Records) or valid Passport

2. Child's Social Security card

3. Immunization record

4. Baptismal/dedication certificate (optional)

5. Completed and signed Enrollment Application, with $100.00 non-refundable application fee

6. Recent report card (for applicant's to Kindergarten through grade 7)

7. Scores on the most recent NYS tests

8. CAT/SAT/Terra Nova scores (Grades 1- 7)
